The Notice of Funding Availability (NOFA) for the Fiscal Year (FY) 2017 Continuum of Care (CoC) Program Competition is now available.  HCPC is the Collaborative Applicant for our CoC and will submit a consolidated application on behalf of the CoC.  Eligible applicants interested in submitting a new project application must submit the application via eSnaps by 5:00 pm EST, August 25, 2017.  HCPC will consider proposals from all eligible organizations, even if those organizations have not previously received CoC Program funding.

New projects must be Permanent Supportive Housing, Rapid Rehousing, or Joint Transitional-Rapid Rehousing as defined by HUD, and must meet all requirements stated in the NOFA.  Applicants are strongly encouraged to read the NOFA in its entirety.  The NOFA and other related information may be found on the HUD Exchange's FY17 CoC Competition NOFA page.

All projects will be evaluated and ranked in accordance with HUD priorities stated in the NOFA and the CoC's approved Performance Measures.  Projects must be located within Polk County.  Submit questions to This email address is being protected from spambots.  2017 Continuum of Care Membership

If you are interested in ending homelessness in Polk County, please join our Continuum of Care! To become a member, you’ll need to complete an application and pay annual dues of $100 for businesses or $50 for individuals. Our membership meets monthly to evaluate existing services and possible service gaps, review state and national guidance, and collaborate on ways to fulfill our mission of making homelessness in Polk County rare, non-recurring, and short-lived. In addition, members have voting rights on CoC policy as well as the opportunity to participate in CoC grant applications.
